Redefining Modern Healthcare Through Personalized, Root-Cause Medicine
Vitality Concierge Medicine was founded on a shared vision to redefine modern healthcare—bringing together advanced medical science, root-cause investigation, and deeply personalized care to help patients achieve lasting vitality and long-term health.
Adam Perlman, MD and Kelley Solosky, APRN combine decades of clinical leadership in integrative, functional, and conventional medicine. Together, they created a practice designed to give patients the time, attention, and expertise needed to truly understand the whole person and develop individualized strategies for optimal health.
By intentionally limiting the number of patients we serve, we are able to provide the time, attention, and thoughtful partnership required to deliver truly personalized care.

Kelley Solosky, ARNP, FNP-C
Clinical Director & Co-Founder
Kelley Solosky is a board-certified Family Nurse Practitioner who trained at the University of Pennsylvania, one of the nation’s leading institutions for advanced medical education. It was there she received her Adult Nurse Practitioner degree. She later completed her Family Nurse Practitioner certification through Thomas Jefferson University, building a strong academic foundation that informs her thoughtful and comprehensive approach to patient care.
With more than two decades in healthcare—including over 17 years practicing as a nurse practitioner—Kelley has worked across a broad range of clinical settings, including family medicine, women’s health, pediatrics, urgent care, and labor and delivery. This diverse experience allows her to approach health from a wide clinical lens, recognizing how interconnected the body’s systems are and how often underlying imbalances contribute to chronic symptoms.
Known for her compassionate and patient-centered style of care, Kelley believes that the most meaningful medicine begins with truly understanding each individual. She takes the time to listen carefully to her patients’ stories, health concerns, and goals, partnering with them to create personalized care plans that support long-term wellness and vitality.
Kelley’s clinical philosophy centers on integrating the strengths of conventional medicine with the investigative tools of functional and integrative medicine. Rather than focusing only on symptoms, she evaluates the body as an interconnected system, working to identify the underlying drivers of imbalance. By combining medical science, lifestyle medicine, and root-cause evaluation, she supports the body’s natural ability to restore balance across the systems that influence health—including hormones, metabolism, nutrition, stress, and lifestyle.
She has pursued extensive postgraduate training in hormone optimization, functional medicine, and mind-body health. Kelley has completed advanced education through the BHRT Training Academy, HRTU Hormone Training Academy, and Elite NP training programs, and is certified in Functional Diagnostic Nutrition (FDN). Her clinical interests include bioidentical hormone replacement therapy (BHRT), testosterone optimization, metabolic health, and personalized strategies to help patients improve energy, resilience, and long-term health.
Together with Dr. Adam Perlman, Kelley co-founded Vitality Concierge Medicine to build a practice centered on time, partnership, and whole-person care. As Clinical Director, she is deeply committed to helping patients restore vitality and achieve lasting health through a personalized, root-cause approach.
